Core Service Concept:

Addressing the major pain points of dementia diagnosis and the prolonged waiting times at public hospital specialist clinics, this project introduces an innovative service model: "Seamless Connection, Filling the Golden Intervention Period". We not only help suspected cases obtain an earlier professional diagnosis but are also dedicated to providing immediate medical and social support services while elderly individuals wait for their specialist appointments. Through this proactive "medical-social integration" intervention, we effectively bridge the service gap, taking action at the early stages of cognitive decline to secure the maximum recovery space for the elderly.

Service Introduction:

Supported by funding from The Hong Kong Jockey Club Charities Trust, the Yan Chai Hospital Social Services Department implements the Jockey Club " Brain Health" Project. Combining clinical medical diagnosis with community rehabilitation support, this project is specifically designed for the elderly who are in the "suspected phase" or "initially diagnosed phase". Understanding that dementia treatment is a race against time, we break the traditional "wait first, serve later" model to ensure that elderly individuals receive professional follow-up and training from the very first day they seek medical assistance.

 

 

Target Service User:

  • Region: Elderly individuals residing in Tsuen Wan, Kwai Tsing, and Tuen Mun districts.
  • Suspected Cases: Individuals suspected of having dementia who are considering or currently waiting for specialist assessments.
  • Diagnosed Families: Families with individuals already diagnosed with dementia who require immediate community support to delay cognitive decline.

 

 

Four Core Professional Advantages:

1. Filling the Golden Gap: Shortening Diagnosis Waiting Time & Immediate Intervention

  • Early & Precise Diagnosis: Through the professional clinical assessments and brain scans provided by the project, suspected cases do not have to wait in uncertainty for public hospital specialist scheduling. They can understand the progression of the condition sooner and begin targeted treatments early.
  • Simultaneous Medical-Social Support: During the "vacuum period" while the elderly wait for government specialist appointments, this project intervenes immediately to provide case management, medication consultation, and non-pharmacological disposal. This ensures that the cognitive functions of the elderly are protected to the greatest extent during the golden intervention period, effectively delaying the rate of decline.

 

2. Dual-Track of Technology and Medicine: From Screening to Diagnosis

  • Convenient Digital Screening: We encourage individuals aged 60 or above to conduct a preliminary self-assessment using the "CUHK Online Cognitive Test" App. Once the test indicates a need for follow-up, the center's team will proactively reach out to provide subsequent support.
  • Comprehensive Medical Examination: This includes clinical assessments, brain imaging scans (MRI/CT), and laboratory blood tests to help rule out other medical causes and precisely diagnose the type of cognitive impairment, laying a scientific foundation for subsequent treatments.

 

3. Diversified Non-Pharmacological Therapies: Reshaping Social and Cognitive Functions

  • Systematic Cognitive Training: Targeted activities designed by occupational therapists—such as sensory stimulation, art creation, and social interaction groups—help the elderly maintain their verbal expression and activities of daily living (ADLs).
  • Enhancing Quality of Life: By participating in structured community activities, patients' feelings of loneliness and anxiety caused by the illness are reduced, allowing the elderly to maintain their dignity and vitality within a familiar community environment.

 

4. Caregiver Empowerment: Professional Guidance and Stress Relief Together

  • Transfer of Caregiving Skills: We organize professional workshops to teach caregivers how to handle behavioral problems and communication barriers. By utilizing assistive tools to improve home care efficiency, we help transform their anxiety into professional caregiving capabilities.
  • Case Manager System: Each participating family is assigned a dedicated case manager who provides long-term, stable emotional support and resource coordination, serving as a steadfast pillar for caregivers on their journey against the illness.
