Health Promotion: Offers a series of free health group activities and seminars for members.
Health Assessment: Provides health risk assessments and enrolment services for the Chronic Disease Co‑Care Pilot Scheme (screening and management of blood glucose, blood pressure, and blood lipids).
Chronic Disease Management, Pain Management & Community Rehabilitation Services: Provides Community Rehabilitation Services, Pain Management Programme and Chronic Disease Patient Empowerment Programme, covering conditions such as stroke, hip fracture, post‑acute myocardial infarction, low back pain and osteoarthritic knee pain.

The core centre is located on the 30th floor of Tower 2, Kowloon Commerce Centre, near Kwai Hing MTR Station, with an area of approximately 1,500 m². It is equipped with a dedicated nurse clinic, consultation rooms, rehabilitation and fitness areas, a self‑service health assessment zone, and multi‑purpose activity rooms.

K&TDHC’s multidisciplinary team comprises nurses, dietitians, pharmacists, physiotherapists, occupational therapists, social workers and fitness coaches. Through health promotion, educational programmes, consultations and counselling, the team enhances public awareness of chronic disease prevention and strengthens self‑management capabilities. Programme themes include healthy diet, weight management, physical activity, cognitive health, fall prevention, mental well-being, sleep hygiene and smoking cessation. The Centre also provides vaccination sessions for seasonal influenza and pneumococcal vaccines to reinforce community immunity.

In addition,four satellite centres and service point have been opened progressively at Cheung Fat Plaza, Shek Yam Shopping Centre, Kwai Shing East Shopping Centre and Cho Yiu Chuen, further extending service coverage across the district.
